Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being caught by the Detente Locking "T": A) As an employee was reaching to grab the bottom mold plate with his left hand the Detente Locking "T" caught his glove, shirt sleeve, and kevlar sleeve. The Starwheel Detente wheel Wrung the employee's arm off 3-4 inches below his elbow. One feasible method of abatement, among others, is to remove the Locking "T" and use a spring loaded tension method to lock the Detente in place.
