Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being caught in/under/between hazards:
a)ACME, 18 Broadway, Browns Mills, NJ, Baling Area: Operating controls and
lights of a Selco baler were not maintained in an operative condition, on
or about
12/05/01.
NOTE:    ABATEMENT DOCUMENTATION REQUIRED FOR THIS ITEM
See page 2, "Notification of Corrective Action" for instructions.
Feasible and acceptable methods for correcting this hazard are:
1.Establish a daily inspection procedure as per the Operating Manual.
2.Provide training to employees on the inspection and use of the baler, as
per
the operating instructions.
3.Make the Operators manual available to all employees who operate the
baler,
including Management as required by the Operator's manual.
4.Follow employer responsibilities for the operations of a baler as per 7.1
Employer Responsibility, ANSI Z245.5-1997.
