Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls to a lower elevation: a)Dynamic Forces, 155 Ninth Avenue, Suite E, Warehouse: Employee weighing 195 pounds, picking or placing product ranging between 5 to 30 pounds, was using a Davidson platform ladder rated at a 200 pound load capacity, exceeding the recommended load rating for the ladder, on or about 1/17/02. Feasible and acceptable methods for correcting this hazard are: 1)Follow manufacturers recommendations posted on the side of the ladder. 2)Use ladders with the proper load capacity for the intended use as per ANSI A14.2- 1990, Section 8.2.1.2. NOTE: ABATEMENT DOCUMENTATION REQUIRED FOR THIS ITEM See page 2, "Notification of Corrective Action" for instructions.
