Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed by a collapsing automotive lift: a)Facility, Service Area, Lift # 13: An employee was working under the lift when it dropped about six inches. b)Facility, Services Area, Lift #14: The structural supports had holes rusted through the front left side of the lift, on or about 4/17/02. Lifts were not tagged out of service when there was a deficiency. c)Facility, Service Area: Employees using automotive lifts were not trained on the proper maintenance, use and inspection procedures for that lift. Feasible and acceptable methods for correcting this hazard are: 1)Establish a periodic preventive maintenance program as per ANSI/ALI ALOIM-2000, Section 6.1.1. 2)Conduct training on the auto lift as per ANSI/ALI ALOIM-2000, Section 4.2 Operator Training. 3)Remove the lift from operation if any deficiencies are note, as per ANSI/ALI ALIOM-2000, 4.5. NOTE:ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM. Attached form or equivalent must be submitted. See page 2,"Notification of Corrective Action" for instructions.
