Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falls: a)Wastequip Accurate, Buidling # 2: A strato lift, serial # 20035, was used without the use of a midrail, on or about 2/8/02. Feasbile and acceptable methods for abating this hazard are: 1)Follow the manufacturer's recommendations for mid-rails. 2)Follow instruction sticker located on the inside of the cage for using mid- rails. 3)ANSI/SIA A 92.3-1990 section 4.9.2.3 - Mid-Rails. 4)Train maintenance Personnel in the training and maintenance of the scissor lift, in accordance with ANSI/SIA A 92.3-1990, section 6.8 - Maintenance Training. 5)Conduct pre-start inspections required by ANSI/SIA A 92.3, section 7.3.3. NOTE:ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM. Attached form or equivalent must be submitted. See page 2,"Notification of Corrective Action" for instructions. b)Wastequip Accurate, Building 2: Employees using personal fall arrest systems were not trained in the proper use, inspection procedures or storage, on or about 2/8/01. One feasible and acceptable method for abating this hazard is for employees to be trained by a competent person to select, inspect, use, store and maintain the equipment based on the manufacturers recommendations and ANSI Z359.1, section 7.3.1. NOTE:ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM. Attached form or equivalent must be submitted. See page 2,"Notification of Corrective Action" for instructions.
