Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were not effectively trained, exposing employees to the hazard of being
caught in or struck by
wood chips, metal fragments, and or metal chipper machine parts:
a)Jobsite - Employee opened chipper disc hood for the Model 100
Brush Bandit Chipper, S/N 436 while chipper disc was in motion.
Feasible and acceptable methods for abating this hazard are:
1.Provide effective training for brush chipper operators.  Training must
include but is not limited to:  identifying danger areas and hazards
associated with equipment and work being performed; ensuring
labeling on equipment is understood by employees.
2.Comply with safety labelling found on the brush chipper which states:
A."DANGER! NEVER OPEN OR CLOSE THE
CHIPPER HOOD WITH THE ENGINE
RUNNING OR THE DISK TURNING.  WAIT
UNTIL THE DISK COMES TO A COMPLETE
STOP!"
B."DO NOT attempt to Operate this Machine
Without First Reading the Operators Manual!"
C."DANGER! NEVER PERFORM ANY TYPE OF
MAINTENANCE ON THIS MACHINE
WITHOUT SHUTTING THE ENGINE OFF
AND REMOVING THE IGNITION KEY."
3.Obtain and follow the manufacturer's procedures in the
Asplundh Brush Bandit Operating & Parts Manual for the
Model 100.
