An employee was fatally injured when he drove a forklift off the edge of a
loading dock. At the
time of the incident, the employee was not wearing the available seatbelt.
On or about 08/30/02.
b) 215 East Broad Street, Paulsboro, NJ 08066 -
Employees were observed operating forklifts on a loading dock while not
wearing the available
seatbelts. On or about 09/11/02.
One feasible and acceptable abatement method is to comply with the
requirements
of ASME
B56.1-1993.5.3.19 and require that employees use the provided restraints
while operating
powered industrial trucks.
