Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) 215 East Broad Street, Paulsboro, NJ 08066 - An employee was fatally injured when he drove a forklift off the edge of a loading dock. At the time of the incident, the employee was not wearing the available seatbelt. On or about 08/30/02. b) 215 East Broad Street, Paulsboro, NJ 08066 - Employees were observed operating forklifts on a loading dock while not wearing the available seatbelts. On or about 09/11/02. One feasible and acceptable abatement method is to comply with the requirements of ASME B56.1-1993.5.3.19 and require that employees use the provided restraints while operating powered industrial trucks. NOTE:ABATEMENT DOCUMENTATION REQUIRED FOR THIS ITEM. See page 2,"Notification of Corrective Action" for instructions.
