Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)  On or about 04-26-00, employees were pulling rods at the Shannan
Lease when after
a transfer by the derrick hand the rods started to travel down the hole
and struck a floor hand
in the head.
Among other methods, one feasible and acceptable abatement method to
crrect this hazard is to
provide a positive latching device, sufficient to hold the elevators shut
under all possible lifting
conditions to prevent the elevators from being sprung open and the rods
frombeing released and
allowed to fall.
