Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to:
a)  On or about 10-03-00 at 723 Coliseum Rd., San Antonio TX an employee
was injured by a mold
that fell off the track/ scissor-lift that the mold was on.  The spacing
of the tracks, was such that they,
allowed the mold to become free of the retaining tracks, if the mold was
pulled from the upper
portion.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to
decrease the distance between the retaining tracks and the mold.  Also the
employer should ensure that
the employees handle the molds from the bottom to lower portion on the
mold to avoid cantilevering
of mold.levering
