General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards in the event of a fall from the moving transfer car: (a).At the Planer Mill - Transfer Car - On or about 10/02/2000, transfer car operators were exposed to the hazard of falling from the cab of the moving transfer car and being struck by or crushed between the transfer car and the concrete surface over which it rode. Among other methods, one feasible and acceptable abatement method would include, but would not be limited to: Installation of a door of substantial strength with a positive means of closure to include an interlocked circuit which would not allow power to be applied for movement of the car unless the door is fully closed and secured. Installation of a power control switch which would require constant pressure applied by the operator and would disengage the power to the drive mechanism when released.
