Three homemade lifting devices fabricated from rusty steel I beams and
pipe, for use
with the forklifts, plus two equalizer beams that were painted yellow, for
use with the
overhead cranes, did not have an approval rating by a certified mechanical
engineer or
identification tag on each unit that specified capacity and identity of
each unit.  Units
were located outside on South end of Storage and Maintenance Area.

Among other methods, feasible and acceptable abatement methods to correct
this hazard include,
but are not limited to:
Purchase of approved lifting devices that are designed for attachment to
forklifts
or overhead

employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
On April 01, 2000, two 1 1/2 ton overhead hoists and one 8,000 pound
capacity forklift,
withimproper attachment of nylon sling eyes on the forks of lift truck
were used to
lift and turn, at a right angle, from North-South, to East-West, an 11,200
pound
beam that was 108 feet long.  There was no qualified responsible person to
be in
charge of the operation and to instruct all personnel involved in the
proper
positioning, rigging of the load, and the movements to be made.  There was
no approved
lifting device attached to the forks and the sling was not properly
attached to the lifting
device so that no sharp edges would contact sling eyes.  Located in the
Glue Forms
Area.
