employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
On February 10, 2000, an employee was utilizing an inadequate cart system
and
procedures to move and handle tall billets of Expandable Polystyrene-EPS
(Styrofoam)
exposing him to the hazards of unstable loads.
