Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: An employee was observed working at a height in excess of 20 feet and fall protection was not in place. Specifically, the employee was standing on a pallet, which was slid onto the forks of a forklift, and personal fall arrest or guardrail was not in place to protect the employee from falling.
