Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees:
a)jobsite, Chastain Walk, 9903 Gray Dove Court, right front--employee was
observed working from an elevated platform of 4x8 sheet of OSB board that
was
supported by a motorized Lull rough terrain forklift without the platform
being
secured to the forks of the forklift to prevent movement or falling.
b)jobsite, Chastain Walk, 9903 Gray Dove Court, right front--employee was
observed working from an elevated platform of 4x8 sheet of OSB board that
was
not provided with guardrails (to include a midrail) and toe boards on all
open
sides.
NOTE:Among other methods, one feasible and acceptable method to correct
this
hazard is to secure a proper work platform (with a toe board and
guardrails to include both a toprail and midrail) to the forks of the
forklift.
