employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees:
TELECOMMUNICATION TOWER CONSTRUCTION SITE, 4501 West Pleasent Valley
Rd.in Parma, Oh.:
On or before April 25, 2000, the employees working on the job site, were
exposed to the
hazards of being caught in moving equipment, striking the tower while
being lowered on the line
without a guideline and falls due to 1) employees being permitted to ride
on top of the ball of
the hoist line and 2) failure to post the load capacities of the gin pole
at the operators station.
Among other methods, feasible and acceptable methods to control the hazard
are:
(1) Conspicuously post rated load capacities of the hoisting gin pole
equipment in sight of the
hoist operator.
(2) Use tag lines to guide and prevent employees from striking the tower
or getting caught in
the lifting block.
(3) Ensure employees attach to the hook under the ball and do not ride the
top of the ball.
(4) Assure CPL 2-1.29 Safe work practices are complied with at all
times.ed
