Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries:
Welding Shop, SSV Area - Welders are exposed to crushing injuries from
wall mounted and
floor mounted jib cranes fabricated in house.  Approximately 27 cranes
were modified and put
into service, but were not analyzed by a qualified person or manufacturer
of the equipment as
required by the American Society of Mechanical Engineers ASME B30.11-1993.
On or about
February 02, 2001, a wall mounted jib crane fell from its supporting
structure and struck an
employee.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to have all cranes and their supporting structures which are fabricated in
house,
analyzed by a
qualified person.  The qualified person would be one who, by possession of
a recognized degree,
or certificate of professional standing, or who, by extensive knowledge,
training and experience,
has successfully demonstrated the ability to solve or resolve problems
relating to the subject
matter and work.r
