Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On July 30, 2002 and times prior thereto, the employer did not ensure that
two propane and two
oxygen compressed gas cylinders located along scrap yard roadways, in
front of the maintenance
shop and the shredder control room door, were firmly secured and insulated
from physical
damage by heavy equipment, including cranes and front end loaders that
routinely use the
roadways.  ANSI standard Z49.1 Safety in Welding and Cutting (1988)
requires compressed gas
cylinders to be chained or otherwise steadied during use to prevent
cylinders from being knocked
over.
Among other methods, feasible and acceptable methods to correct the
hazards can be as follows:
a.Chain or otherwise secure compressed gas cylinders in a metal frame; or
b.Relocate torch cutting operation to an area away from roadways and secure
cylinders in an upright position.
