Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: a. Receive serious injuries due to ceiling hanging lamp detached from base and support only from two fluorescent tube of eight feet long and twelve feet two inches in height above a lower level, located in front of cooler in a storage area. The employee was supplying merchandise. Among other methods, one feasible and acceptable abatement method to correct this hazard is install the fluorescent lamp in accordance to National Electrical Code-1996, Sec. 410-15(a). a. Recibir lesiones serias dado a lampara de techo colgante separada de la base y apoyada solamente de dos tubos fluorescentes de ocho pies largo y doce pies dos pulgadas de altura sobre el nivel mas bajo, localizado frente a la nevera en area de almacen. La empleada estaba supliendo mercancia. Entre otros metodos, un metodo aceptable y posible de corregir este riesgo es instalar la lampara fluorescente de acuerdo al Codigo Electrico Nacional-1996, Sec. 410-15(a).

Receive injuries due to dis-orientation at the automatic safety system from cardboard compactor. At the time of inspection the system was not functioning and the employees were using it. The cardboard compactor, trade mark Vertical Baler, serial number BV6030B was located in the storage area in the supermarket. Among other methods, one feasible and acceptable abatement method to correct this hazard is fix the safety system to machine so that it can not deactivate. a. Recibir lesiones por la des-orientacion del sistema de seguridad automatico de apisonadora de carton. Al momento de la inspeccion, el sistema no estaba funcionando y los empleados estaban usandola. La apisonadora de carton, marca Vertical Baler, Serie# BV6030B estaba localizada en el area de almacen en el supermercado. Entre otros metodos, un metodo aceptable y posible de corregir este riesgo es fijar el sistema de seguridad a la maquina de forma tal que no se pueda desactivar.o.

Receive injuries due to broken glass (28.5 inches long by 22.5 inches wide at 43 inches high from floor level) in a door at rest area in the supermarket. The employees were using it to enter to the rest area. Among other methods, one feasible and acceptable abatement method to correct this hazard is change the broken glass to a new appropiate safety glass. a. Recibir lesiones dado a cristal roto (28.5 pulgs. largo por 22.5 pulgs. ancho a 43 pulgs.de alto del nivel del piso) en la puerta del area de descanso en el supermercado. Los empleados estaban usandola para entrar al area de descanso. Entre otros metodos, un metodo aceptable y posible de corregir este riesgo es cambiar el cristal roto por un cristal de seguridad nuevo apropiado.
