Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
a. Receive injuries due to failure of the hoisting and lowering structure,
equipped with
a load-carrying unit; linked with chain to an overhead electric chain
hoist, model RRT,
serial #L-0600NX, 3 ton cap.; used by employees as an elevator to lift
merchandise,
where employees walked into load-carrying unit platform to perform
load/unload
operations, was not certified as to comply with requirements specified in
ASME A17.1-
1990 and PR Special Rule #5. Among other methods, one feasible and
acceptable
abatement method to correct this hazard is the construction of an elevator
that meet the
requirements in ASME A17.1-1990 and PR Special Rule #5.
A. Recibir lesiones debido a fallo de estructura para subir y bajar,
equipado con una
unidad de acarreo de carga; unido con cadena a una cabria electrica de
cadena de tope,
modelo RRT, serie L-0600NX, 3 tons cap.; usado por empleados como elevador
para
subir mercancia, donde empleados entran a plataforma de unidad de acarreo
de carga a
realizar operaciones de carga/descarga, no estaba certificado en
cumplimiento con los
requerimientos especificados en ASME A17.1-1990 y Regla Especial #5 de PR.
Entre
otros metodos, un metodo acceptable y factible para corregir este riesgo
es la
construccion de un elevador que cumpla con los requisitos de ASME
A17.1-1990 y la
Regla Especial #5 de PR.

estaban expuestos
a:
a. Receive injuries due to mechanism failure of overhead chain electric
hoist, model
RRT, 3 tons capacity, serial #L-0600NX, 2HP; used as lifting mechanism for
a load-
carrying unit, that employees used to transport merchandise. At the time
of
the
inspection, the hoist was not inspected. The hoist was located at 2nd
floor, north side of
building. Among other methods, one feasible and acceptable abatement
method to correct
this hazard is to performed periodic inspections as defined in ANSI
B30.16-1973 sec.16-
2.2.1.3 (or later edition).

electrica de cadena,
modelo RRT,3 tons capacidad, serie #L-0600NX, 2HP; usada como mecanismo de
izar
para levantar unidad de acarreo de carga, que empleados usan para
transportar
mercancia. Al momento de la inspeccioo, la cabria no estaba inspeccionada.
La cabria
estaba ubicada en el 2do. piso, lado norte edificio. Entre otros metodos,
un metodo
aceptable y posible para corregir este riesgo es realizar inspecciones
periodicas, segun
define en ANSI B30-16-1973, sec.16-2.2.1.3.
