Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: a. Receive injuries or fractures due to failure of a chain hoist electric, trademark CM Loadstar, 3 Tons.,that was used by employees for handling goods. The chain hoist electric was not inspected and was located in a warehouse building at the establishment. Among other methods, one feasible and acceptable abatement method to correct this hazard is to performed periodic inspection as defined in ANSI B30.16-1973, Sec. 16- 2.2.1.3, page 15. a. Recibir heridas o fracturas debido a falla de la cabria electrica de cadena, marca CM Loadstar, 3 tons., que era usada por los empleados para manejo de mercancia. La cabria electrica de cadena no estaba inspeccionada y estaba localizada en el edificio de almacen en el establecimiento. Entre otros metodos, un metodo aceptable y posible de corregir este riesgo es realizar inspecciones periodicas segun se define en ANSI B30.16-1973, Sec. 16-2.2.1.3., pagina 15.

Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: a. Receive injuries or fractures due to mechanism failure of a chain hoist electric , trademark CM Loadstar, 3 tons., that was not recomended to be used for handling personnel, according to ANSI B30.16-1973, Sec. 16-2.3.2.3. (e), page 20. Carrying unit was used by employees for handling goods and personnel. The chain hoist electric was located at the establishment, warehouse of building. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install an elevator according to disposition of SPECIAL RULE #5 of the OCCUPATIONAL SAFETY AND HEALTH ACT. a. Recibir lesiones o fracturas debido a falla de la cabria electrica de cadena, marca CM Loadstar, 3 Tons. que no era recomendada para izar personas de acuerdo ANSI B30.16- 1973, Sec. 16-2.3.2.3. (e), pagina 20. La unidad de carga era usada por los empleados para manejo de mercancia y personal. La cabria electrica de cadena estaba localizada en el establecimiento, edificio de almacen. Entre otros metodos, un metodo aceptable y posible de recorregir este riesgo es instalar un elevador segun las disposiciones de la REGLA ESPECIAL #5 de la LEY DE SEGURIDAD Y SALUD OCUPACIONAL.
