SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A. Employer knew or should have known that employees working from work
platform not secured to lifting carriage or forks of a Sky Track forklift
serial number 24680 located on the front, right side of building on the
Piedmont-East Urgent Care Center jobsite were exposed to hazard of being
thrown from work platform.  A feasible and useful means of abatement, among
others, is to secure work platform to lifting carriage or forks of
forklift.
B. Employer knew or should have known that employees working from work
platform elevated at heights in excess of 10 feet attached to a Sky Track
forklift serial number 24680 not attended by operator at the Piedmont East
Urgent Care Center were exposed to the hazard of the forklift moving and
employees falling from work platform.  A feasible and useful means of
abatement among others is to assure that the operator remain at controls of
the forklift while employees are elevated in the work platform.
