A.  Employer knew or should have known that operator of Lull Highlander II
forklift not at
the controls while employees were elevated in work basket, in excess of 10
feet high,
constituted a hazard of lift, shifting or employees falling, being struck
by objects, or other
hazards and no way for employees to get down.  A feasible and useful
method to correct this
hazard, among others, is to require the operator to remain at the controls
of the forklift while
employees are in the work basket.
