recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,

A.  Employer knew or should have known that employees working near the
International 18-Wheeler truck with tag number CG29056 which had an
inoperable
back up alarm and no observer used when backing up were exposed to the
hazard
of being struck by the truck when it is operated in reverse gear.  A
feasible
and useful means of abatement, among others, is to repair backup alarm on
the
International 18-Wheeler truck located in the yard of the maintenance shop
or
use on observer.
