in excess of
17 feet in plywood platform box on forks of Case 586G forklift, Equipment
No. 700053, on
south side of steeple structure were exposed to falling due to the box not
being secured to the
forks or mast.  A feasible and useful method to correct this hazard, among
other methods, is
to secure the box to the forks or mast.
