572802, Serial Number P3673B43, forklift operated in reverse gear with
view obstructed to
rear with no automatic reverse signal alarm and no observer used
constituted a hazard of
running over employees with no warning.  A feasible and useful method to
correct this
hazard, among other methods, is to install an automatic operable reverse
signal alarm.
