Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) Plant, Northeast Side- The Ballymore Company manually propelled
elevating aerial platform
(scissor lift), serial number 2692 and model HW4-24 had two bent
outriggers and one outrigger
stuck in position.  The hydraulic power for the lift mechanism allowed the
lift to descend too
quickly. Maintenance employees were exposed to the hazard(s) of falls from
the lift during the
operation/use of the aerial platform for maintenance work in the plant.
Among other methods, one feasible and acceptable method to correct this
hazard is to
include, but not be limited to the following:
Conform to The American National Standard Institute "Manually Propelled
Elevating
Aerial
Platforms" ANSI A92.3-1990, including conducting frequent and annual
inspections of the aerial
platform and maintenance of the operating and maintenance manuals for the
lift.
Abatement documentation is required for this item in accordance with 29
CFR 1903.19
paragraph (d).
