safe and healthful
for employees, and did not do every other thing reasonably necessary to
protect the life, safety,
and health of the employees:
(a)  The general contractor, Schuler Homes of Oregon did not ensure that
the sub-contractor at
the above site had a safe work environment as evidenced by: unguarded
windows approximately
12 feet from ground to sill, stair ways with out hand rails, lumber and
debris in front of the
access doors, forklift operator not certified.
