ORS654.010 The employer did not furnish a safe place of employment which
was safe and
healthful for employees therein, and did not furnish and use such devises
and safeguards, and
did not adopt or use such practices, means, methods, operations, or
processes as would be
responsibly necessary to render such employment and place of employment,
safe and
healthful, and did not do every other thing reasonably necessary to
protect the life, safety and
health of such employees, as evidenced by;
1) did not ensure that all employees while working on a pump-jack scaffold
system 14 feet 3
inches above ground level, were protected from falling, by using some type
of fall protection
system:
2) for the employees who were using the second story window as a means of
access and
egress from the platform of the pump-jack scaffold system, were exposed to
a potential fall
of 14 feet 3 inches:
3) that the employees using the temp power supply to supply electricity to
various power
tools, were protected from electrical shock by using either GFCI's, or an
assured equipment
grounding conductor program.
THIS VIOLATION WAS COMPLIED WITH AT THE TIME OF INSPECTION.
