Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputations while working with and around the freight elevator: a). At 3647 S. Santa Fe Drive, Englewood,CO: Warehouse Storage Area, the freight elevator was not maintained in accordance with ASME A17.1-1996 Section 204.2b in that, the equipment hoist was not enclosed to protect personnel from pinch point hazards. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to maintain the freight elevator in accordance with ASME A17.1-1996. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
