Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: a)At a well site owned and operated by Prime Operating Company, a derrick hand was fatally injured when he fell approximately 108 feet with the derrick after guywire anchors "deadman" pulled loose causing the derrick to collapse. Prime Operating did not perform a pull test or any other appropriate test to verify and record the capacity of the anchors. b)At a well site owned and operated by Prime Operating Company, a derrick hand was fatally injured when he fell approximately 108 feet with the derrick after guywire anchors "deadman" pulled loose causing the derrick to collapse. Prime Operating did not insure the anchors were installed in such a manner that liquids drained away from anchor shafts. ***Certification and Documentation of Abatement Required***on
