Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
1)  Banbury Pit area; On 03/19/01 employees were using a 4 ft. metal pipe
commonly known
as a "cheater bar" to provide additional leverage on a 2 ton U.S capacity
Harrington Hoist Inc
come-along to attempt to extract a 593 lb load of rubber from beneath
Banbury 271.  This
exposed employees to the recognized hazard of being struck by equipment
should the cheater bar
overload the come-along and slings.
A feasible and acceptable abatement method to correct this hazard would
be:  Discontinue the
use of the "cheater bar" on a come-along and insure all system components
used in removing
rubber from beneath a Banbury are rated well above the rated capacity of
the
come-along (ex:
slings).
2)  Banbury Pit Area; On 03/19/01 employees untrained in the use of a
manually lever operated
chain hoist were using a 2 ton U.S capacity Harrington Hoist Inc
come-along with a "cheater
bar" and anchored to a rough edged eyelit on a motor housing with a 1" x
45" nylon web sling
to pull a 593 lb of rubber from beneath Banbury 272 exposing employees to
the hazards of being
struck by equipment should the cheater bar overload the come-along and
slings.
A feasible and acceptable abatement method to correct this hazard would
be:  Train employees
to be familiar with the operating controls of the hoist, the warnings of
the hoist, the hoisting
practices listed in Section 21-1.7.2 of ASME B30.21-1994 as Addenda to
ASME B30.21c-1998,
and the operator's manual provided by the hoist manufacturer.
3)  Banbury Pit area; On 03/9/01 employees used a Lift Hunter 1" x 45"
nylon
web sling run
through a 2 1/2" rough edged eyelit on the motor housing of Banbury 271 to
anchor a 2 ton U.S
capacity Harrington Hoist Inc come-along.  Subsequently, the Lift Hunter
nylon sling failed
under the pulling pressure causing the Harrington Hoist come-along to fly
back and strike an
employee in the chest.
A feasible and acceptable method to correct this hazard would be:  To not
run nylon slings over
rough edged surfaces or pad the rough edged surface.
