Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Two chain anchor attach pins in use on the Hyster Model H225H forklift while lifting a 50-foot I-beam broke causing the I-beam to fall 20-25 feet to the concrete floor and striking an employee. Among other methods, one feasible and acceptable method to correct this hazard is to disassemble, inspect and lubricate these pins using the manufacturer's recommended lubricant or an equivalent substance. ***Certification and Documentation of Abatement Required***on
