Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the recognized hazard of being struck by
high-pressure drilling-
fluid hoses:
a. Above the drilling platform; the pump end of the rotary hose was
subject to come
loose and fall upon employees on the drilling platform.  One feasible and
acceptable
method to abate this hazard is to secure this end of the rotary hose to
the derrick with
a chain or wire rope.
In the mud pump area; the high-pressure mud-pump hoses were not secured to
prevent them from whipping in the event they came loose.  One feasible and
acceptable method to correct this hazard is to secure the ends of these
hoses with a
wire rope or chain.

the catline
around the cathead:
On the drilling platform at the draw works; employees used the catline and
cathead to lift
objects onto the drilling platform; they were subject to be crushed or
otherwise injured.  One
feasible and acceptable method to correct this hazard is to ensure that a
qualified person is at
the controls to immediately stop the cathead if necessary.

In the pipe rack area; employee(s) worked inside the pipe racks as joints
of casing were
being rolled onto the catwalk for placement in the well; the pipe racks
were not level;
employee(s) were subject to be crushed by casing rolling from a higher
position to a lower
one.  One feasible and acceptable method to correct this hazard is to
ensure that the pipe
racks are level and to move the casing from the ends of the joint while
standing outside the
pipe racks.

safely escape in the event of a fire or other emergency:
An employee worked in the derrick as drill pipe and collars were removed
from the well in
preparation to run surface casing into the well; a fire or other emergency
could have resulted
from a blowout, a leak and ignition of diesel fuel or other event.  One
feasible and
acceptable method to abate this hazard is to install an auxiliary means of
escape prior to
personnel working in the derrick.
