(2)At the SH-4 & SW 149th St. jobsite in Tuttle, OK Muskogee Bridge Company did not ensure the gate opening was effectively closed on the work platform on the Skytrak 10042 exposing the employees to the recognized hazards of falling 17-18 ft. (3)At the SH-4 & SW 149th St. jobsite in Tuttle, OK Muskogee Bridge Company did not install toeboards on the work platform on the Skytrak 10042 exposing employees to the recognized hazards of falling objects. ***Certification and Documentation of Abatement Required***on
