Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were subject to be struck by violently whipping wire rope or caught by looping wire rope: a.On Rig 26 on July 31, 2001; the wire rope sand line was corroded and had been mechanically damaged by improper winding on the drum; employees were subject to be struck by the whipping sand line in the event it parted. One feasible and acceptable method to abate this hazard is to implement a wire rope inspection, maintenance and replacement program. b.On August 1, 2001 on the Forster #1 lease; the towing attachment on the 1975 Chevrolet pick-up truck was used in an attempt to hoist the swab and attached wire rope sand line out of the well; the towing attachment had previously failed and was known to be defective; employees were subject to be struck by whipping wire rope or caught by looping wire rope. One feasible and acceptable method to correct this hazard is to ensure that all components of a hoisting system are substantially constructed, maintained in safe condition and not loaded in excess of their design capacity. c.On July 31, 2001 on Rig 26; employee(s) worked in front of the sand drum to guide the wire rope sand line off of the drum as the swab went into the well; tension was subject to be momentarily removed from the sand line allowing it to loop around the employee which could have resulted in the employee being crushed or a body part being severed when tension was restored. One feasible and acceptable method to abate this hazard is to properly install the sand line on the drum so that employees would not have to work in front of it. ***Certification and Documentation of Abatement Required***on
