press brake did not have a guard over the foot pedal to prevent accidental
activation of the press brake.
ANSI Standard B11.3-1982, paragraph 4.2.4.1.4 states "When a mechanical
foot pedal
is furnished with the power press brake, a means shall be provided for
preventing
accidental operation of the power press brake."
One feasible and acceptable abatement method, among others, to correct
this hazard is
to put a gover guard over the foot pedal.
