recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fall hazards from an elevated personnel platform:
(a)TJM Builders at 1540 Platte Court Loveland, CO 80538:  On and before 07-
26-00, the employer did not ensure that a metal, job-made personnel
platform
used in conjunction with a John Deer forklift (Model #JD 480) was properly
designed, constructed, secured, utilized and maintained.  Condition exposed
employees to a fall hazard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to design, construct, utilize and maintain the
personnel
platform in accordance with ASME/ANSI B56.6-1992, Safety Standard for
Rough Terrain Forklift Trucks.
