Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a: Hazard of being struck by high pressure water and pieces of the Garlock Gasket Style 3400 gasket material: In the testing area, employees were conducting hydrostatic testing of a pressure vessel and utilized test pressures that exceeded manufacturer's and ASME recommendation for the type of gasket used. Methods of abatement include maintaining test pressures at or below the pressure rating of the gasket or use gaskets with higher pressure ratings as discussed in ASME B31.1 - 1998 Edition, Section 108.4 Gaskets, Table 112 (i), General Notes: (e).
