Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with improperly installed auxiliary escape lines: The auxiliary escape line from the derrickman's working platform was rigged in an unsafe manner: a.The escape line was fastened through a hole in the side of a 55-gallon barrel used as an anchorage point. b.The 55-gallon barrel anchorage point was removed and a non-augered, straight metal stake equipped with an eye plate was installed as an anchorage point. c.The escape line was installed such that it did not have the recommended 6 to 12-foot sag; employees did not have the 20 to 25 feet of travel distance prior to reaching the ground anchorage point in the event of an emergency. Among other methods, one feasible and acceptable abatement method to correct the hazard is to thoroughly train all employees involved in rigging up in the correct and recommended methods of installing the auxiliary escape line. Such procedures are discussed in API Recommended Practice 54, section 6.10, 3d. Edition, August 1999, American Petroleum Institute. Also, a booklet, Geronimo Operators Manual, Geronimo Manufacturing, discusses hazards of incorrectly installed auxiliary escape devices.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a collapsed mast (derrick) hazard: Rig #2 was vulnerable to a collapse of the mast (derrick) during drilling operations. The four wire ropes used to guy the rig were connected to turnbuckles by incorrect mounting and use Crosby-type clips. Among other methods, one feasible and acceptable abatement method to correct the hazard is to properly mount and use the Crosby-type wire rope clips.
