Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an inhalation hazard: At this plant, the employer did not ensure that its ammonia refrigeration system adhered to the guidelines of the ANSI/ASHRAE Standard 15 (1994) - Safety Code for Mechanical Refrigeration. Examples of observed deviations from these recommendations for safe operation included the following: Section 7.2: Liquid ammonia piping for coolers at the ceiling level inside work rooms was exposed to possible accidental damage by forklifts handling stacked goods onto storage shelf spaces; Section 8.13.2: Each engine room was not equipped with an ammonia gas detector to actuate an alarm and mechanical ventilation; Section 8.14(i): Remote control of the mechanical equipment in the engine room was not provided immediately outside the engine rooms. Control switch for the ventilation fans was not provided immediately outside the engine room door; Section 11.6: A self-contained breathing apparatus was not located outside, but close to, the engine rooms; Section 11.8: Emergency procedures were not posted outside the engine rooms, immediately adjacent to each door.
