Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards associated with improperly installed auxiliary
escape lines:
The auxiliary escape line from the derrickman's working platform was
rigged in
an unsafe manner:
a.The escape line was fastened to a concrete block used as an anchorage
point.
b.The escape line was installed such that it did not have the recommended
6 to 12-foot sag; employees did not have 20 to 25 feet of travel distance
prior to reaching the ground anchorage point in the event of an
emergency.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is
to thoroughly train all employees involved in rigging up in the correct
and recommended
methods of installing the auxiliary escape line.  Such procedures are
discussed in API
Recommended Practice 54, section 6.10, 3d. Edition, August 1999, American
Petroleum
Institute.  Also, a booklet, Geronimo Operators Manual, Geronimo
Manufacturing, discusses
hazards of incorrectly installed auxiliary escape devices.
