EXAMPLE:  Employees are exposed to a workplace violence hazard that is
likely to
cause death or serious physical harm.  An employee was injured when caring
for
a patient in the hospital.  The patient is mentally ill.  The patient had
an
aggressive episode in which the employee was assaulted by the patient.
During
this incident, the patient attempted to gouge out the employee's eyes.  The
patient continued to assault the employee, which resulted in an eye injury
and
broken ribs.n
