Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a.) Wash Bay/Break Room/Office - Employee(s) were potentially exposed to fire and explosion hazards from the ignition of natural gas because the gas line (All Trade Commercial Grade 3/8-inch ID synthetic hose) for the office heater was not installed correctly. The gas line was routed from the heater in the wash bay across the ceiling and through the wall of the break room. In the break room, the gas line was spliced and routed through the wall into the office. The gas line was then joined to a small heater mounted on the wall of the office. The entire run of the flexible hose was approximately 15 - 20 feet. All of the connections and splices were accomplished with spider clamps (circular ring with screws) that were not approved/listed. The hose utilized to convey the gas was not listed/approved as evidenced by the lack of applicable ASTM markings. The National Fuel Gas Code The fittings utilized to join the hose must also be listed/approved. These connectors consist of compressed fittings. ANSI Z223.1 Part 5: * Paragraph 5.5.1 requires the hose/flexible tubing utilized to convey the gas must be listed/approved for use with natural gas. * Paragraph 5.5.2 prohibits flexible hose utilized for gas distribution to pass through walls, pass from one room to another, and be a minimum of 6-feet in length. FEASIBLE METHODS OF ABATEMENT: ANSI Z223.1 Paragraph 5.5.3 only permits the use of approved/listed flexible hose to be connected to the building's gas piping system if the gas utilization equipment requires mobility or is subject to vibration. In lieu of utilizing a flexible hose: 1. Replace the rubber hose with approved piping that is secured with hangers of adequate quality/strength. 2. Manual gas shut-off valves must be plainly marked and within 6-feet of the gas utilization equipment. 3. Fittings utilized to join the piping must be listed and approved and installed in accordance with the National Fuel Gas Code.
