Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were potentially exposed to fire and explosion hazards from the ignition of natural gas leaks because a flexible gas supply hose and manual shut-off valve were not configured properly: a.) Forklift Maintenance Area Truck Dock Bay #4 - A space heating unit was being utilized to heat the Truck Dock Bay and was connected to the building's natural gas piping via a flexible hose. From the natural gas valve on the wall of the bay by the stairs, the flexible gas supply hose was routed along the wall, then down the wall, and across the floor to the unit. The flexible gas supply hose and manual shut-off valve were not installed in accordance with the National Fuel Gas Code and ANSI 10.10 - 1990 in that: (1) The manual shut-off valve on the wall was not equipped with a handle or other equally effective means to shut-off the flow of gas (2) The flexible gas supply hose was not listed/labeled for use with natural gas (3) The portion of the flexible gas supply hose that was routed across the floor was not protected from damage which could potentially be caused by forklift tines and/or tires. ABATEMENT NOTE: Among other methods, feasible and acceptable abatement methods to correct the hazard are to comply with applicable requirements noted in ANSI A10.10-1990 ~ Safety Requirements for Spaced heating Devices - Section 7 & NFPA 54/ANSI Z223.1-1999 ~ National Fuel Gas Code Section 5.5 Manual Shut-off Valve: National Fuel Gas Code (NFPA 54/ANSI Z223.1) Sections 5.5.4 requires the manual shut-off valve to be plainly marked and located within 6-feet of the equipment served. Hose & Hose Connections: National Fuel Gas Code (NFPA 54/ANSI Z223.1) Sections 5.5.1 & 5.5.2 requires gas utilization equipment to be connected to the buildings piping with listed hose and hose connectors. ANSI A10.10 Section 7.3.11 requires hoses and fittings to be the correct type for the fuel that is utilized by the heating unit. National Fuel Gas Code (NFPA 54/ANSI Z223.1) Sections 5.5.2 & 5.5.3 requires gas hose to be the minium practical length and not pass through walls, partitions, ceilings, or floors. ANSI A10.10 Section 7.3.9 & 7.3.16 requires the hose and fittings to be protected against damage and located to minimize damage. ANSI A10.10 Section 7.3.10 requires the total run of the hose to be short as practicable and not exceed 25-feet. ANSI A10.10 Section 7.3.15 prohibits the used of slip-end connectors and ring keepers. ANSI A10.10 Section 7.3.14 requires the hose to be securely connected to fittings via a mechanical method. ANSI A10.10 Section 7.3.13 requires all hose connectors to be able to withstand a pull test of 400 pounds without experiencing leakage.
