Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to material handling equipment collisions, caught-in-between
hazards, and struck-by hazards from the operation of material handling
equipment in low visibility conditions:
a.) Owens-Illinois, Plant 19/Cullet Dump Area: On or about Jan 1, 2003,
the Komatsu
front-end loader and Yale forklift B9 collied in the cullet dump area
while both pieces of
equipment were dumping and maneuvering furnace cullet during start-up
operations.
Steam was created by the furnace cullet reacting with the cold atmosphere.
The Komatsu
WA-180 front-end loader was operated in steam clouds that created whiteout
like
conditions.  The front-end loader operator training program did not cover
the unique or
potentially hazardous environmental conditions associated with operating
the
loader in an
area enveloped by steam.
ABATEMENT NOTE:
Among other methods, feasible and acceptable methods to correct the
condition include:
(1) Utilize two-way radio headsets to establish communication between the
forklift
operator and front-end loader operator.
(2)  Train front-end loader operators on the rules, techniques, and
hazards associated with
operating the loader in reduced visibility conditions created by steam,
dust, and/or fog
(3) Erect warning signs, stanchions, and barricades to keep pedestrians
out of the area
