Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to falling crane load(s):
a)  The Terex-RO 15-ton hydraulic truck crane (serial number 45907981915)
had an improperly
attached main load line (the dead end of the rope was clipped to the live
end in a wedge socket) and
the dead end of the rope showed damage evident of two-blocking.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to comply with
the manufacturer's specifications and instructions, and to follow ANSI
B30.5, both of which have specific
requirements for wedge socket installation and anti-two-blocking.
Abatement certification required within 10 days after abatement date.  The
certification shall include a
statement that abatement is complete, the date and method of abatement,
and state that employees and their
representatives were informed of this abatement.
