Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to a fall hazard:
a) National Guard Armory site - Employees worked from a two-point
suspended scaffold
rigged with one single and one double CROSBY-WESTERN painters supply
block.  The
double block hooks were attached to the roof irons and the single block
hooks were
attached to the scaffold stirrups.  None of the hooks were equipped with
safety latches
nor were the hook openings secured by wire or other means to prevent the
hooks from
becoming disengaged from their attachment points.  (on or about 9/18/01)
ABATEMENT NOTE:  Among others, one feasible and acceptable abatement
method is to
purchase blocks equipped with safety latches on the hooks.
Abatement certification and documentation required within 10 days after
abatement date.
The certification shall include a statement that abatement is complete,
date and method of
abatement, and states employees and their representatives were informed of
this abatement.
Abatement documentation shall  include documents demonstrating that
abatement is
complete, such as evidence of the purchase or repair of equipment,
photographic or video
evidence of abatement or other written records.
