Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
House of Mufflers and Brakes, 5815 Center St - The employer did not
furnish employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to:
Employees are exposed to potential crushing hazards while working around
and under the
automotive hoists located in the shop service area.  Not all of the
automotive lifts were equipped
with functional safety devices nor maintained and inspected in accordance
with the
manufacturer's recommendations.  The safety device on the lift in Bay 4
was not automatically
functioning due to alterations of the safety device.  The employer was not
following maintenance
and care procedures of the lift, as designated by the manufacturer, as
evidenced by the inability
of the manager to provide the manufacturer books to the CSHO and the lack
of functional safety
devices.  The employer was not conducting scheduled inspections of the
lifts
to ensure that all
components are in safe operating condition, as recommended by the
Automotive Lift institute,
Inc, ALOIM - 1994 Standard Approved as an American National Standard by
American National
Standards Institute.
a)Qualified Inspection Documentation - Conduct lift inspections following
the
specific directions provided by the lift manufacturer in accordance with
section
5.6.1 of ANSI/ALI ALOIM-1994.
b)Inspection Points-Mechanical Lifts - Check chains and report excessive
wear on
links pins or side plates, deformed, bent, rusted or broken links, or
presence of
foreign material in accordance with section 5.6.4.2 of ANSI/ALI ALOIM-1994.
c)Preventive Maintenance Procedure - Establish a periodic preventive
maintenance
procedure in accordance with the recommendations of the lift manufacturer
in
order to ensure the continued safe operation of the lift in accordance
with section
6.1.1 of ANSI/ALI ALOIM-1994.
d)Preventive Maintenance Documentation - Maintain a record of preventive
maintenance procedures performed.  Note specific checks made, parts
replaced,
adjustments made, results of any measurements and recommendations made in
accordance with section 6.1.4 of ANSI/ALI ALOIM-1994.
e)Preventive Maintenance Frequency - Follow the lift manufacturer
recommendations for frequency of preventive maintenance in accordance with
section 6.1.5 of ANSI/ALI ALOIM-1994.
f)Repair Maintenance Documentation - Maintain a record of each repair
maintenance procedure performed in accordance with section 6.1.5 of
ANSI/ALI
ALOIM 1994.
g)Modifications - Do not make modifications or reconstruct any automotive
lifts
without the express written permission of the manufacturer performed in
accordance with section 7. of ANSI/ALI ALOIM 1994.
Abatement certification and abatement documentation is required for this
violation. The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful. The abatement
certification sheet is enclosed with the citations.ion
