Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)Building 50 - Poduction machinist operating the Leblond lathe to machine part #20270 was exposed to the flying chips and coolant of the Lucas horizontal mill with the Employer ID# 22-751---342. The employer did not provide a shield and/or guard either fixed or portable, which is specified in ANSI B11.6 1984 section 5.5. This violation was noted on or about 04-09-2001.2.
