Section 88.4 - Code of Iowa (2001) 88.4: Employer did not furnish to each of the employer's employees employment and a place of employment which is free from recognized hazards that are causing or are likely to cause death or serious physical harm to employee in that employees were exposed to: (a)S&H Solid Waste - The company installed a new hydraulic cylinder on a piece of mobile equipment. The hook and cable were not properly placed in the manufacturers designated bracket when not in use. This condition exposes employees to hazards, such as amputations and/or lacerations when the hook is placed on the same grab bar the employee uses to steady himself when the truck is moving. Among other methods, one feasible and acceptable abatement method to correct this hazard is to train and supervise employees to use the manufacturers designated bracket that came with the truck. ANSI Z-245.1 - 1984 6.1 This violation was noted on or about 4-3-01.

This exposes employees to hazards, such as amputations and/or lacerations or other physical harm if this control is operated with out knowing its function. Among other methods, one feasible and acceptable abatement method to correct this hazard is to label the control function in a conspicuous place. ANSI 2-245.1 - 1984, 7.1.4.1
